How do I add a Pre-Appointment questionnaire to a service?

A pre-appointment questionnaire is a questionnaire you can send to your patients XX hours before the time of appointment.

An example could be to send a "COVID Symptom Screener" 24 hours before they have an in-clinic appointment with you.


To update a service with a pre-appointment questionnaire:

  • Sign in to your Hashealth account
  • Navigate to the Services menu item on the left

If you are creating a new service or editing an existing the steps are the same:

  • Click on the dropdown under "Pre-appointment questionnaire"
  • Select the questionnaire you want to add to the service
  • You can preview it.
  • Click save once you are finished
